Information Release Forms

In the waivers and release section of the education abroad program application or insurance enrollment there is a FERPA release form completed. Ohio State will only use that information if a traveler is:

  • unable to speak for themselves
  • perceived to be a danger to themselves or others
  • fail to comply with legitimate directives of university officials
  • in a location directly impacted by a significant health, safety or security incident such as civil unrest, act of terrorism or significant natural disaster.

In all other instances, we expect travelers to maintain contact with their family unless a specific request has been made for the international risk manager or other university staff to be in contact.

To grant a parent, guardian, or school official with access to student claims data or other medical information managed through GeoBlue, you should complete the HIPAA release form found on your GeoBlue account.
