The Ohio State University Office of International Affairs Passport Acceptance Facility is happy to assist with any questions you may have regarding passport applications. Please be aware that the Passport Acceptance Facility is inside of the Office of International Affairs. The office can be visited through the main entrance up a flight of stairs or through the north side hallway ramp by calling the phone number above. If you need to use the ramp entrance, please plan ahead to call so staff can provide entry through this secure entrance.

FAQ

Do I have to schedule an appointment, or can I walk in?

Walk-in appointments will not be accepted. Please schedule an appointment online. If you are scheduling an appointment for more than one person, indicate that on the form. 

How does payment work?

When you apply in person at a passport acceptance facility, you will need to pay two fees: one to the U.S. Department of State and one to the facility where you submit your application.

To pay the passport application fee:

  • Submit a check (personal, certified, cashier’s, traveler’s) or money order payable to “U.S. Department of State” with your application.
  • Write the applicant’s name and date of birth in the memo section.

To pay the passport acceptance fee and photograph fee:

  • Submit a check or money order payable to "The Ohio State University."
  • Pay with a credit card. 
Do you take passport photos on-site, or do I need to bring my own?

Passport photo services are available for a fee. If you would like to bring your own, refer to the U.S. Department of State website. 

Can I fill out and sign my application form (Form DS-11) before I arrive?

While you should fill out the form ahead of time using black ink, you must not sign it. The acceptance agent is legally required to witness you signing the document under oath.

If I am bringing my original birth certificate and physical Driver’s License, do I still need to bring photocopies of them?

Yes. The facility has to mail a physical copy of your ID and citizenship evidence to the State Department along with your application. The copies must be single-sided, on standard 8.5" x 11" white paper.
