Share your vaccination information with the university

Ohio State needs everyone's help as we plan public health measures for the fall.

Those who have received the COVID-19 vaccine, should share their vaccination information with the university to be counted in the university's overall vaccination rate.

  • Students can report through My BuckMD. Fully vaccinated students who have reported their status will be tested less frequently (once a month or less), while other students will be tested at least once a week.
  • Faculty and staff can report through this simple online form.

Those who can be vaccinated but haven’t gotten their shots yet, please be vaccinated as soon as possible—and then report your status. Vaccinations not only protect individuals—it enhances the safety of the whole community.

Ohio State is collecting vaccination data to support our public health measures for the fall.

Sharing statuses will allow us to further inform decision-making about public health guidelines and to better support underrepresented portions of our community. As more of our community is vaccinated, Ohio State plans to continue to relax mandates — but we need good data to make these decisions.

Even for those who were vaccinated through the Wexner Medical Center or if it is displayed in My BuckMD or MyChart, the university is requesting that you voluntarily share the information through the online form.
